Thank you for visiting our codeReady website and for reviewing our Privacy Policy.

Personal information for Create a Plan or Kit

None of the information you provide when you create a plan or kit is saved by codeReady. This includes your name and address, plus any medical or contact information you choose to add to your plan. This information is securely transmitted to the codeReady servers, and is retained only for the duration of your visit. Once you close your browser, the information is gone. We recommend that you save your plan or kit to your computer, print it, or email it to yourself to retain a copy.

Email address in codeReady

You may choose to provide your email address in order to receive specific information from the codeReady site, such as an emailed copy of your plan or an email response from the codeReady site. Your email address will not be retained by codeReady in a database, and will not be reused or sold. However, as a general point of information, please realize that anytime an email is sent, all servers that email passes through do log the transaction, including sending and receiving address, date, time and subject. The content, including attachments such as your plan, is not logged.

Email address to GovDelivery

You may choose to sign up with the GovDelivery notification service, in order to receive weekly email tips. If you sign up, your email will be stored in a database, and used for this purpose only. It will not be reused or sold. GovDelivery's privacy policy is available online for more information.

General information about your visit

When you browse through any website, certain information about your visit is collected, such as:

  • the name of the domain you use to access the Internet, but not your specific email address
  • the date and time of your visit
  • the browser and operating system you're using
  • the pages you visited
  • the address of the website you came from when you came to visit

We use this information for statistical purposes and to help us make our site more useful to visitors. Minnesota law describes this information as "electronic access data." Please see the Public Disclosure section of this policy for more information on electronic access data.

Personal Information — a Legal Description

"Personal information" is information about a natural person that is readily identifiable to that specific individual. Personal information includes such things as an individual's name, address, and phone number.

Personal information is required if you choose to create a plan or kit on the codeReady site. However, this information is not saved. It is retained for the duration of your browser session, after which you must save, print, or email the plan to yourself to keep the data.

Personal information is also collected on the codeReady feedback form and Order a Kit page. We use this information only as necessary to respond to your communication. If you choose to provide this information, State law and the federal Privacy Act of 1974 may protect it. However, this information is a public record once you provide it, and may be subject to public inspection and copying if not protected by Federal or State law.

Cookies

Most of the codeReady site does not use "cookies." There are several exceptions to this: creating a plan or kit, Google Analytics, and the GovDelivery subscription service.

Cookies are used on the Create a Plan pages and the Create a Kit pages to assign a unique identifier to your session while you are creating your plan or kit. The cookie is not shared with other websites, does not store personal information, and expires as soon as you close your browser.

Cookies are also used by Google Analytics, the web metric program which collects aggregate information about the codeReady site. This information helps us make the site work better for you. Google Analytics does not collect personal information about you, simply general information about your visit, as described above.

Cookies are also used by the GovDelivery subscription service, which allows you to subscribe to certain information in order to receive an e-mail alert when it is changed. The GovDelivery subscription service uses "session cookies", which are stored on your computer only until you close your browser. The session cookie allows the server software to keep you logged in as you browse GovDelivery. The cookie is not shared with other websites, does not track or store personal information, and expires as soon as you close the browser. If you choose not to accept a cookie on your computer, you will not be able to create a custom plan or kit. You can still use templates to create your own plan and kit, and you can access the rest of the site.

Public Disclosure Laws

In the state of Minnesota, laws exist to ensure that government is open and that the public has a right to access appropriate records and information possessed by State government. At the same time, there are exceptions to the public's right to access public records that serve various needs including the privacy of individuals. Exceptions are provided by both State and Federal laws.

Under Minnesota Statutes section 13.15, electronic access data are not available for public inspection or copying. Your electronic access data may be used by Minnesota Department of Health staff whose job assignments reasonably require access, by the Minnesota Commissioner of Administration for the purpose of evaluating electronic government services, to other government entities to prevent unlawful intrusions into government electronic systems, or as otherwise provided by law.

All information collected at this site becomes public records that may be subject to inspection and copying by the public, unless an exemption in law exists. Minnesota Statutes Chapter 13 and Minnesota Rules Chapter 1205 define Minnesota's Government Data Practices Act.

Security, Intrusion, Detection Language

The Minnesota Department of Health, as developer and manager of the codeReady site, has taken several steps to safeguard the integrity of its telecommunications and computing infrastructure, including but not limited to authentication, monitoring, auditing, and encryption. Security measures have been integrated into the design, implementation and day-to-day practices of the entire MDH operating environment as part of its continuing commitment to risk management. This information should not be construed in any way as giving business, legal, or other advice, or warranting as fail proof, the security of information provided via the MDH Web site.

Unauthorized attempts to deny service, upload information, change information, or to attempt to access a non-public site from this service are strictly prohibited and may be punishable under Title 18 of the U.S. Code, including the Computer Fraud and Abuse Act of 1986 and the National Information Infrastructure Protection Act.
